PATIENT REQUIREMENTS
- Patient must be homebound and requires skilled services
- Services must be medically necessary and intermittent
- Care plan must be overseen by a primary physicial.
- Referrals to Home Health Services can be provided by the physician, family, friends, insurance company, nursing home, rehab units and even patients.
